GENERAL SUMMARY OF DUTIES

The Nurse Navigator (NN) will perform nursing assessment and triage of patient care needs – both in-person and remote – and recommend interventions when clinically appropriate. The NN will work collaboratively with Portland Gastroenterology Center (PGC) physicians, clinical, and administrative staff to support effective and efficient operations of Portland Gastroenterology Center’s Provider Professional Clinic (PPC) and its functions.

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Serves as point of contact for referring physicians, patients, and caregivers to assist with patient scheduling and coordination of care.
  • Coordinates appointments with providers to ensure timely delivery of diagnostic and treatment services and ensures that appropriate medical records are available for these appointments.
  • Facilitates linkages to follow-up services.
  • Assists physicians and Advanced Practice Providers (APPs), including conducting patient assessments.
  • Works closely with the appropriate departments to provide access to pharmaceutical samples when appropriate.
  • Maintains communication with patients, families and/or medical designeesand health care providers to monitor their satisfaction with their experience with PGC.
  • Receives patient telephone calls and intervene appropriately for that patient’s specific needs in a timely manner. Alert physician or APP as needed to patient needs that require physician intervention.
  • Follows up with patients as needed by telephone, to ensure compliance with treatment guidelines and to evaluate changing or uncertain clinical situations.
  • Arranges language translation and/or interpretation services as needed.
  • Maintains patient care standards and assures that patient care functions, such as assessment, treatment and documentation are completed safely and effectively.
  • Participates in interdisciplinary team conferences, in-services and continuing education programs, national and regional professional organizations, and community-sponsored programs specific to Gastroenterology and digestive health.
  • Incorporates evidence-based research into clinical practice and provide recommendations on practice clinical policies and processes to practice Directors.
  • Coordinates patient care responsibilities within the scope of the practice for licensed and unlicensed health care providers.
  • Builds partnerships with local agencies and groups (referrals to other services and/or support groups).
  • Perform medical procedures as needed.
  • Demonstrates respect for life, dignity, and rights of all persons.
  • Completes special assignments or projects as needed.

QUALIFICATIONS

EDUCATION AND/OR EXPERIENCE
  • Associate’s degree in Nursing from an accredited institution required, Bachelor’s degree preferred; and
  • Two years’ professional nursing experience; or
  • Equivalent combination of education and experience.
CERTIFICATES, LICENSES, REGISTRATIONS
  • Active Maine Registered Nursing License required
  • Active BLS & ACLS certification required (can be obtained within six months)

  • Proven knowledge of GI practice strategies.
  • Proven organizational, communication, and interpersonal skills.
  • Proactive initiative to identify issues and recommend solutions.

  • Knowledge of the nature of diseases, their treatments both curative and palliative, and the side effects of related therapies.
  • Knowledge of area agencies and willingness to collaborate with these agencies, initiating referrals when applicable.
