Overview
This position provides medical care and treatment services with emphasis on chronic disease management and promotion of health and palliative care for patients not best served in a clinic setting. Care is provided in patient residencies and through audio and video sessions. This role may focus on regular care or relief care depending on needs.
NotesWe are accepting applications for future openings. Roles could be 32 to 40 hours per week based on candidate needs. Service areas may include Multnomah, Washington, Clackamas, Clark, Clatsop, Columbia, and Marion counties. Service area and schedule preferences can be discussed with the Hiring Manager.
Essential ResponsibilitiesClinical Service
- Provide diagnosis, treatment, consultation, and follow-up care for patients, including management of critical, prolonged, and long-term health issues.
- Perform comprehensive health assessments including physical and psychosocial history; identify actual and potential health needs based on medical, mental/emotional, financial, and social factors.
- Make referrals to appropriate social or medical agencies and practitioners.
- Identify, evaluate, and address prevention of diseases, maintenance of chronic diseases, and appropriate end-of-life care per goals of care outlined by the patient and family.
- Develop and implement proactive patient-centered care plans with the care team.
- Order testing and medications with appropriate follow-up and reassessment; perform some surgical procedures as appropriate under certification.
- Approve medication refills and home health orders; respond to caregiver requests to ensure continuity of care.
- Provide compassionate anticipatory guidance through decline due to chronic disease and deliver high-quality palliative care aligned with patient and family goals.
- Educate patients and families on goal-based, appropriate preventative care, chronic care, urgent care needs, and prescribed medications and treatments.
- Develop and maintain relationships with lay and professional caregivers and care facilities to ensure continuity of care and proper implementation of orders.
- Deliver quality care while maintaining productivity metrics and practice within applicable license scope and legal/regulatory requirements.
- Participate in after-hours on-call schedule.
Administration
- Communicate effectively with medical staff on consultation, collaboration, and referrals.
- Determine need for specialty and rehabilitative services and make appropriate referrals.
- Oversee care plan and case management activities for assigned patients.
- Attend and participate in required staff meetings and committees; provide coverage for colleagues as needed.
- Participate in quality improvement activities and maintain medical records according to agency protocols and quality metrics.
Experience and/or Education
- Master’s degree from an accredited advanced nurse practitioner program
- Licensed nurse practitioner in the state of Oregon (ANCC or AANP certified)
- Drug Enforcement Administration (DEA) license
- Valid driver’s license, acceptable driving record, and automobile liability coverage or access to an insured vehicle
- CPR certification at hire or within 6 months of position
- Minimum 2 years of related clinical experience

- General medicine principles, diagnosing and treating diseases and injuries
- Current literature and ethical/medico-legal aspects of medical practice
- Public health concepts, epidemiology, health services delivery and financing, and environmental health
- Population served in adult health, including culture, demographics, barriers to care, and special needs
- Clinical quality improvement and medical charting familiarity
Skills and Abilities
- Proficient with electronic systems, including audio and video supported encounters
- Medical diagnostic and treatment capabilities
