We are seeking a dedicated and compassionate Registered Nurse (RN) to provide in-home healthcare services to patients. The ideal candidate will deliver high-quality nursing care, ensure patient safety, and support recovery in the comfort of the patient's home. This role requires strong clinical skills, autonomy, and experience with OASIS documentation.
Key Responsibilities- Deliver patient-centered nursing care in home health settings.
- Complete comprehensive assessments using OASIS documentation standards.
- Develop, implement, and evaluate individualized care plans.
- Collaborate with interdisciplinary teams to coordinate patient care.
- Monitor and document patient progress using EPIC electronic health record system.
- Educate patients and families on treatment plans, medications, and health maintenance.
- Maintain accurate and timely documentation in accordance with regulatory requirements.
- Active state Registered Nurse (RN) license
- Basic Life Support (BLS) certification
- OASIS documentation experience (required)
- Proficiency with EPIC charting system
- Ability to work independently in home health environments
- Strong clinical judgment and time management skills
- Must have at least 2 years of recent experience in the specialty within the past 2 years
- Prior experience with Client systems or patients
