Position: Licensed Practical Nurse (LPN) - NOC Shift

The Licensed Practical Nurse (LPN) is responsible for monitoring residents' health, administering medications and treatments, assisting with daily care needs, and maintaining accurate medical records. This role supports the healthcare team in delivering safe, compassionate, and high-quality care while ensuring compliance with clinical standards, policies, and regulations. The LPN assists with resident health observations and reports changes in condition and helps implement individualized Service Plans and Treatment Service Plans (TSPs) to support resident well-being and independence.

The LPN collaborates with staff to coordinate care and maintain effective communication. In addition, the NOC Shift LPN is the Manager on Duty and will report concerns and operational issues to the appropriate on-call staff.

Duties and Responsibilities Care Coordination
  • Assist with resident Health Evaluations for development and updates of Service Plans.
  • Provide assistance with instrumental activities of daily living (ADLs), environmental orientation, assistance with medication, treatments, and other care while encouraging self-care and independence to Residents in an Assisted Living Facility with Special Needs Contract.
  • Deliver services as indicated with each resident treatment and service plans (TSP).
  • Conduct regular observations of residents and update Services Plan Agreements for all assisted living residents in accordance with state guidelines.
  • Accurately document resident care using Point Click Care  and other electronic health record systems.
  • Perform neurological checks following resident falls and report findings according to facility protocols.
  • Maintain effective communication with pharmacies, physicians, home health agencies, and other healthcare providers.
  • Carries out quality assurance reviews from the electronic medical record and other data sources.
  • Acts as a liaison between assisted living resident, family, and physician to coordinate the care needs of each resident within the boundaries of assisted living nursing assistance per Oregon Administrative Rules (OAR) standards and requirements.
  • Participates in the maintenance of assisted living resident's charts including weekly skin observations and Outside Coordination Notes and reviews documentation performed by staff on Provider/Pharmacy/3rd Party Provider Notes and documentation.
  • Process orders and assist with the development and updates of Treatment and Service Plans (TSPs).
  • Triage needs and concerns and respond appropriately to urgent situations in accordance with Oregon Board of Nursing best practice and standards.
  • Provide support and backup to Direct Care Staff as needed, including medication administration when staffing needs require.
  • Coordinate monitoring the weights of each assisted living resident and identify significant weight losses or gains to RN, for intervention planning.
  • Encourage and support resident participation in behavioral health services, meetings, and care programs.
  • Initiate home health services when appropriate and coordinate communication between the resident, physician, and home health agency.
  • Follow the regulations and guidelines as set forth by the state of Oregon.
Other Duties
  • NOC Shift LPN is the Manager on Duty and will report concerns and operational issues to the appropriate on-call staff.
  • Manage the self‑medication assessment process in accordance with Oregon state guidelines and facility policies.
  • Conduct quality assurance reviews utilizing electronic medical records and other available data sources.
  • Audit medication carts and medication overflow supplies to ensure organization, compliance, and accuracy.
  • Remove expired medications and coordinate appropriate replacement according to pharmacy and facility procedures.
  • Apply direction change stickers to medications when orders are updated to ensure accurate administration.
  • Coordinate and complete cycle fills with the pharmacy to maintain appropriate medication inventory. Assist with projects as assigned and perform other related duties.
  • Perform other related duties as needed to ensure safe and effective resident care.
