Overview

As a member of the Senior Community Care (SCC) team, you will provide care to patients at home, nursing homes and assisted living settings across Multnomah and Clackamas Counties, OR. The role is full-time (Monday‑Friday, 8:00am‑5:00pm) and involves traveling to various facilities. You will maintain a work‑life balance with no on‑call, weekend, or holiday duties.

Primary Responsibilities
  • Conduct comprehensive assessments.
  • Manage members through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, and interventions.
  • Manage members during a short-term sub-acute rehabilitation episode.
  • Ensure accurate and complete documentation, including ICD-10 codes.
  • Communicate and collaborate with the interdisciplinary care team.
  • Conduct advanced illness and advanced care planning conversations.
  • Provide patients and caregivers with counseling and education.
Required Qualifications
  • For Nurse Practitioners:
    • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
    • Active and unrestricted Nurse Practitioner license in the state of residence, or ability to obtain by start date.
    • Board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with certification in at least one of the following specialties:
      • Family Nurse Practitioner
      • Adult Nurse Practitioner
      • Gerontology Nurse Practitioner
      • Adult‑Gerontology Acute Care Nurse Practitioner
      • Adult‑Gerontology Primary Care Nurse Practitioner
  • For Physician Assistants:
    • Graduate of an accredited Physician Assistant degree program.
    • Active and unrestricted Physician Assistant license in the state of residence, or ability to obtain by start date.
    • Board certified by the National Commission on Certification of Physician Assistants (NCCPA).
  • Current, active DEA licensure or prescriptive authority, or ability to obtain post‑hire as required by state regulations.
  • Ability to gain a collaborative practice agreement, if applicable in your state.
  • Ability to complete physical requirements of the job, including moving a 30‑pound bag in and out of the car, navigating stairs and a variety of dwelling conditions, and remaining in a stationary position to perform physical assessment.
  • Driver's license and access to reliable transportation.
Preferred Qualifications
  • 1+ years of clinical experience in practice, preferably in long‑term care.
  • Experience meeting the medical needs of patients with complex behavioral, social, and/or functional needs.
  • Understanding of geriatrics and chronic illness.
  • Understanding of advanced illness and end‑of‑life discussions.
  • Proficient computer skills, including documentation of medical information in written and electronic medical records.

Pay is based on several factors including local labor markets, education, work experience, and certifications. In addition to your salary, we offer benefits such as a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contributions. The salary range for this role is from $91,700 to $163,700 annually based on full‑time employment.
