Central City Concern is seeking a dedicated Primary Care Provider with a passion for addiction medicine and serving people in crisis. Join a collaborative, multidisciplinary team at Hooper Detox Center, where every decision and every interaction supports individuals taking their first steps toward recovery.
This is more than just a clinical role - it's a frontline opportunity to provide transformative care to people experiencing homelessness, substance use disorders, and serious mental illness. You'll work alongside our experienced Medical Director to deliver evidence-based withdrawal management, with a focus on trauma-informed care, dignity, and connection.

At Hooper, patients arrive in crisis. You'll use your clinical skills from performing physical exams to initiating Buprenorphine - help stabilize their health. In this role, you will:
* Conduct physical exams and oversee withdrawal management protocols
* Provide ongoing care and crisis response in a fast-paced detox setting
* Collaborate with a compassionate team of nurses, case managers, and behavioral health providers
* Help shape recovery pathways for some of our community's most vulnerable members

Location:
Hooper Detox Center (1535 N Williams Portland, OR 97227)
Schedule:
Morning shift 8am
-12pm Afternoon shift 1pm-5p
This position can be a .6 fte (24hrs)
-1.0 fte (40hrs) a week position pending the Providers licensure/certifications and experience.
Salary Ranges (Full-Time) starting at:
Nurse Practitioner $135,000 annually
Physician Assistant $135,000 annually
Naturopath $142,000 annually
Physician (MD/DO) $231,000 annually
MINIMUM QUALIFICATIONS:
NP/PA
* Graduate of an accredited Nurse Practitioner Program or Physician Assistant program required of all Nurse Practitioner applicants.
* If candidate is a Nurse Practitioner, graduate of an accredited school of nursing and has unrestricted licensure to practice medicine in Oregon or eligibility for such licensure.
* Current Advanced Practice Registered Nurse (APRN) Licensure with National certification or Licensed to practice as a Physician's Assistant in Oregon, with at least 2 years' experience.
MINIMUM QUALIFICATIONS:
MD/DO/ND
* Board eligibility or certification in Internal Medicine or Family Practice.
* Graduation from a CNME-accredited, four-year Naturopathic Medical program.
* Must have completed a CNME-certified primary care residency program.
* Licensed naturopathic physician in the State of Oregon.
* Maintain an Active DEA Certification.
MINIMUM QUALIFICATIONS:
ALL APPLICANTS
* A strong background in mental health and addiction medicine is highly preferred, as well as experience working with unhoused and underserved/marginalized populations.
* Must have full prescriptive powers in Oregon with a DEA number.
* Demonstrated skills in primary care medicine with a minimum of 2 years' experience preferred.
* Must have current BLS certification.
* Available to work during standard Hooper Center hours.
* Must meet CCC privileging requirements as required by FTCA.
* Available to work during standard Hooper Center hours.
* The OAR prohibits A&D program staff from having, within the previous 3 years a DUI conviction or diversion, a felony conviction, or a conviction under the Uniform Controlled Substances Act.

* Physical ability to bend, stoop, kneel, squat, twist, reach, pull, and lift moderately heavy objects.
* Able to climb stairs several times a day.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
Essential Clinical and Compliance Duties:
* Familiarity with site specific standing orders, workflows, communication standards.
* Provides admission orders for new patients into residential care, including orders for the administration of medications for opioid use disorder.
