Transplant Coordinator RN - Heart Transplant and VAD at Providence St. Vincent Medical Center
This full‑time (1.0 FTE), day shift position provides coordination and case management for ventricular assist device (VAD) and cardiac transplant patients. The Coordinator collaborates with the program medical director and referring physicians to promote optimal patient outcomes, supports clinical and technical aspects of VAD or transplant patient care, facilitates care conferences and multidisciplinary committee reviews, and shares 24‑hour call coverage. They also coordinate patient care through medical procedures, ongoing education, inpatient and outpatient care delivery, and follow‑up.
Required Qualifications- Education and licensing that meet RN certification requirements.
- Oregon Registered Nurse license upon hire.
- Minimum 2 years of VAD experience.
- Minimum 3 years of clinical experience with VAD patient care.
