POSITION SUMMARY
The Psychiatric Mental Health Nurse Practitioner (PMHNP) provides psychiatric evaluations, treatment plans and ongoing medication management & psychotherapeutic support to patients enrolled in Fora's Residential and Outpatient programs. The PMHNP also provides treatment services in our low barrier MAT clinic and inpatient Withdrawal Management Unit.
SCHEDULE is four 10 hour shifts, including one weekend day
Focus and Specific Tasks:
Provides Addiction Medicine Services
- Completes a Biopsychosocial assessment for patients entering MAT services
- Admits patients to inpatient Withdrawal Management Unit
- Identifies a treatment plan specific to patient's substance use pattern consistent with evidence-based practice and with patient's goals of care
- Provides both in-patient and telehealth follow up care for MAT patients and administers injections (Sublocade, Brixadi, Vivitrol)
- Coordinates with case managers, peers & nurses to provide wrap around care
- Practices harm reduction principles in all aspects of patient care
- Refers patients for any acute or chronic medical needs to primary or specialty care services
Provides psychiatric medication management
- Completes psychiatric evaluations with dual diagnosis patients who are referred for assessment, prioritizing patients according to their need and acuity
- Makes recommendations for psychiatric medication management based on evidence-based practices and patient goals
- Collaborates and consults with interdisciplinary treatment team about mental health status of patients
- Educates patients on general health guidelines and preventative measures to improve over-all health and decrease risk factors
- Communicates accurate medication orders to the pharmacy
- Orders and evaluates diagnostic tests when appropriate
- Enters accurate medication and provider orders into the EMR
- Supports patients in following through with referrals to other appropriate resources to meet mental health needs
- Collaborates with outside providers as needed in order to optimize care
- Document assessment, planning, implementation and evaluation of interventions in the patient records
- Provides periodic on-call coverage, supporting on-site nursing staff from home, usually one week per month
- Demonstrate ethical behaviors by adhering to established professional codes of ethics that define the professional context within which the counselor works, in order to maintain professional standards and safeguard the patient.
- Adhere to Federal and State laws and agency policies and procedures regarding the treatment of substance use disorders.
- Recognize the importance of individual differences that influence patient behavior and apply this understanding to clinical practice.
- Maintains healthy boundaries with patients and their families.
- Attend staff meetings as scheduled.
- Additional trainings to be completed as assigned in a timely manner.
- Special projects and other duties as assigned.
- Participate in ongoing supervision and consultation.
- Provide leadership to the teams in such a way that the work environment supports and encourages values of the agency: honesty, empathy, respect, inclusion, unity.
- Proactively identify barriers to effective medication administration systems and works with residential and medical staff to address them.
- Maintain quality treatment services that are responsive to client needs and reflect best practices.
- Participates in quality improvement meetings and initiatives
- Maintains and strengthens good working relationships with members of the clinical teams
- Communicates and coordinates with staff from other departments effectively.
- Maintains cleanliness and safety of work environment
- Performs related tasks as assigned
- Develops policies and procedures as needed
- Establish a therapeutic environment in which staff works together so that patients' needs, individual circumstances, and culture determine the optimal clinical approach.
- The PMHNP will maintain and keep current all credentials and continuing education as required by local, state and federal authorities. Furthermore, the nurse practitioner will independently stay informed of evidence-based practices in the field of addiction psychiatry medicine.
This position does not have supervisory duties.
Requirements
QUALIFICATIONS
EDUCATION AND EXPERIENCE REQUIRED
- Master level nursing degree.
- Current licensing/certification as a Psychiatric Mental Health Nurse Practitioner.
- Current BLS CPR & First Aid Certification
SKILLS AND ABILITIES
REQUIRED
- Proficient in Microsoft Office Suite, specifically Outlook, Excel and Word
- Detail-oriented and organized
- Ability to maintain professional boundaries with patients, the community, and other staff members.
- Ability to work collaboratively with patients, other treatment program staff, team members, and to treat everyone with respect and dignity at all times.
