Position: Registered Nurse - Hospice 7/7

Overview

The Hospice RN is responsible for the delivery of professional nursing services to hospice patients as indicated by the plan of care developed by the interdisciplinary team. Responsibilities include initial and ongoing assessment of patients, developing and revising the plan of care, timely documentation, and communication of assessments, planning and interventions, coordination of care among team members.

Essential Responsibilities
  • Perform initial and ongoing nursing evaluations of patients, families, and caregivers and assist in developing and implementing the plan of care.
  • Case management (including admissions, initial hospice assessments, routine or focused nursing visits, telephonic triage, night/weekend coverage, and coverage for increased census or staffing shortages).
  • Coordinate and monitor all care, keep the physician and other team members from agency and community resources apprised of changes in patient condition, and adjust care as needed to improve quality of life.
  • Use clinical assessment skills and best practices to identify and provide effective care for patients, families, and caregivers.
  • Actively participate in IDT meetings for development and evaluation of patient plans of care.
  • Develop and implement a plan of care for each assigned patient/family/caregiver unit, and evaluate that plan.
  • Assist families in determining goals of care and completing Advance Directives and POLST forms as needed.
  • Provide dietary counseling as needed.
  • Advocate for patients and families, facilitate meetings, and collaborate with IDT during crisis management.
  • Provide information regarding psychosocial, spiritual, physical, and financial needs to the IDT.
  • Serve as liaison with hospitals, care facilities, and agencies to promote interagency cooperation and facilitate referrals.
  • Participate in continuing education and orientation of other team members.
  • Collaborate with Housecall Providers program staff to enhance continuity of care.
  • Document in electronic medical records accurately, completely, and in compliance with regulations, certification standards, legal and ethical standards.
  • Perform duties in compliance with Medicare Condition of Participation.
  • Maintain confidentiality in compliance with HIPAA laws.
  • Coordinate hospice care in accordance with professional standards of practice.
  • Practice within the scope of license and law.
  • Maintain professional boundaries with patients, families, and caregivers.
  • Maintain agreed work schedule and meet deadlines for work completion.
  • Function in various care environments (home, skilled nursing facilities, assisted living).
  • Serve as an ambassador for Care Oregon and Housecall Providers.
  • Demonstrate cooperation and teamwork with all staff and community partners.
  • Participate in orientation of new employees, volunteers, or students.
  • Attend staff and mandatory in‑service meetings.
  • Assume responsibility for updating professional skills and ongoing education.
  • Be familiar and comfortable with a person‑centered approach to communication, education, and care planning.
  • Maintain patient and personal safety regarding occupational health and safety.
  • Participate in Quality Assessment – Performance Improvement efforts.
Experience and/or Education Required
  • Current unencumbered license as a registered nurse in the State of Oregon.
  • Minimum two years of RN medical/surgical experience in an acute care setting, and/or a combination of medical/surgical, home health, hospice or long‑term care experience.
  • One year of experience in quality process improvement/QAPI may substitute for one year of RN experience.
  • CPR certification at hire or within six months.
  • Valid driver’s license with acceptable driving record and vehicle coverage or access to an insured vehicle.
Preferred
  • Bachelor of Nursing degree or higher.
  • Certified in Hospice and Palliative Care (CHPN).
  • Three to five years in hospice triage, case management, admissions, or night/weekend roles.
