Family Nurse Practitioner (FNP) / PMHNP
Position located at Halsey Plaza NE, Portland, Oregon 97220. Operations Monday–Saturday with a flexible schedule to accommodate family needs.
Hours:
8 AM–6 PM. Full time.
Sign‑on bonus: $10,000 after 90 full days. Compensation: 1–3 years of experience – $125 / hour; 4–7 years – $140 / hour; 5+ years – $160 / hour. Bonuses paid per volume of work (production or fee‑for‑service).
Job SummaryThe Family Nurse Practitioner specializes in comprehensive healthcare for individuals and families across the lifespan, providing primary care and mental health services. The FNP will be licensed and able to practice independently while collaborating with physicians, nurses, and other health‑care professionals.
Key Responsibilities- Conduct comprehensive health and mental‑health assessments and formulate diagnoses.
- Order and interpret diagnostic tests to support diagnosis.
- Develop, implement, and modify evidence‑based treatment plans.
- Prescribe medications and monitor therapeutic outcomes.
- Provide health‑promotion, disease‑prevention, and lifestyle education.
- Conduct routine screenings and immunizations.
- Collaborate with physicians, nurses, other health‑care and mental‑health professionals.
- Maintain accurate electronic health records and documentation.
- Participate in interdisciplinary meetings, case conferences, and quality‑improvement initiatives.
- Perform case management for all established patients, including case closeout.
- Engage in continuing education and stay current on best practices.
