Description
Transplant Coordinator RN - Heart Transplant and VAD at Providence St. Vincent Medical Center. This is a Per Diem/On-Call, Day Shift position, with Call Schedule on Evenings and Weekends as available.
This position will provide coordination and case management for ventricular assist devices (VAD) and cardiac transplant patients in collaboration with the program medical director and referring physicians to promote optimal patient outcomes. Responsible for clinical and technical support for ventricular assist devices or cardiac transplant patients regarding evaluation, implant management, post follow‑up and clinical training. Effectively uses care conferencing and multidisciplinary transplant/VAD committee for patient evaluations and clinical reviews.
Facilitates patient care within an outpatient clinic and shares 24 hour call. Will coordinate patient care through medical procedures, ongoing education, inpatient and outpatient care delivery and follow‑up.
- Education to meet certification, license or registration requirement.
- Upon hire:
Oregon Registered Nurse License - 2 years of VAD experience.
- 3 years Clinical experience with VAD patient care.

CompensationPosted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on‑call, overtime, premiums, extra shift incentives, or bonus opportunities.
