Overview
The RN Stroke Navigator will be the first contact the patient before hospital discharge and then follow the patient after discharge providing coordination between the patient, PCP, neurologist and other specialists. This position will perform in hospital early post discharge screening for medication compliance, healthy lifestyle, depression, medical complications, and caregiver burden while all the while being the patients advocate and anticipating their needs.
Required Qualifications- Bachelor's Degree in Nursing; or obtained within 3 years.

Oregon Registered Nurse License - Upon hire:
National Provider BLS - American Heart Association - 5 years of related neurological clinical experience.
- Master's Degree in Nursing.
- Upon hire: NIHSS Certification
- Upon hire:
Stroke Certified Registered Nurse - Upon hire:
American Association of Neuroscience Nursing membership and certification - 1 year of experience in an Ambulatory setting.
- Experience working with patients who have neurological conditions, and their families.
- Experience working with a Joint Commission Certified Stroke Center.
