The Resident Care Manager (RCM) LPN or RN is responsible for overseeing the care and services provided to a designated group of residents in accordance with the care plan and regulatory requirements. This position works closely with the Director of Nursing in serving as a clinical leader and liaison between nursing staff, physicians, residents, and families to ensure quality, person-centered care.
RESPONSIBILITIES- Meet and assist with new admissions.
- Discuss problems or concerns with residents, family members, staff, and other interdisciplinary care team members.
- Read the 24-hour report for changes of condition to determine assessments, plans, notifications, and consults needed.
- Review psychoactive, restraint, and restorative programs.
- Keep residents’ family and physician informed of any changes in resident behavior, medications, or conditions of health.
- Coordinate all aspects of care for assigned residents, including assessment, care planning, implementation, and evaluation.
- Monitor and ensure timely and accurate completion of MDS assessments and care plans in accordance with CMS/Oregon state guidelines.
- Supervise and guide nursing staff (CNAs, LPNs/RNs) providing direct care to ensure best practices and compliance with facility standards.
- Collaborate with physicians, therapists, social workers, and families to ensure effective resident care planning.
- Conduct resident and family care conferences and maintain ongoing communication.
- Participate in Quality Assurance/Performance Improvement (QAPI) efforts and survey readiness.
- Monitor changes in resident condition and ensure timely interventions and documentation.
Train, mentor, and support nursing team members to promote high standards of resident care. - Assist with incident investigations and ensure follow-up and reporting.
- Ensure compliance with infection control protocols, safety practices, and HIPAA regulations.
- Other duties as assigned.
- Current Oregon RN or LPN license in good standing (Required).
- Minimum 3–5 years of experience in long-term care or skilled nursing setting (Preferred).
- Prior leadership, supervisory, or charge nurse experience (Preferred).
- Strong understanding of MDS, care planning, and Oregon long-term care regulations.
- Excellent communication, organizational, and leadership skills.
- Ability to work effectively in a team environment and manage multiple priorities.
