• Graduate of an approved school for practical nurses
• Upon hire:
  Oregon Licensed Practical Nurse
• Upon hire:
  National Provider BLS – American Heart Association
• 1 year of LPN experience

• Coursework/Training: IV therapy course and competency demonstration
