Focused Ultrasound RN Patient Coordinator
The Focused Ultrasound RN Patient Coordinator provides compassionate, evidence-based, and efficient care to individuals, families, communities, and patient populations. The role is to be a face of the focused ultrasound program, will be the primary liaison and primary point of contact for focused ultrasound patients and their families. The Patient Coordinator will deliver patient information and education to new and existing patients, and manages the patient experience from initial inquiry through follow-up consultations.
This position interacts with patients via multiple channels (e.g. phone, email, text) to educate, motivate, and provide consultation. The Patient Coordinator will handle all inbound and outbound communication to/from existing and new patients. The Patient Coordinator's care delivery is consistent with the Oregon Nurse Practice Act, the American Nurses Association (ANA) Scope and Standards of Practice, and the ANA Code of Ethics.
- Patient Coordination. Plan, coordinate, and carry-out all patient logistics with regards to the focused ultrasound program. Orient and educate patients and their families; explaining the focused ultrasound treatment; initiating the care plan; providing educational information in conjunction with direct care providers related to treatments, procedures, medications, and continuing care requirements. Maintain professional and technical knowledge of focused ultrasound technology, treated conditions, and alternate treatments.
Guide patients from time of initial inquiry through post-op. Act as single point of contact for patients: scheduling, screening, outreach, follow-up. Responds to patient inquires and concerns ensuring patient satisfaction and direct follow-up. Schedule patient appointments and make reminder and preparation calls. - Scheduling & Authorization. Coordinate Schedule patient CT and MRI scans; provide outside imaging centers proper scanning protocols on behalf of patients. Maintain knowledge of Medicare and Commercial Payors. Obtain patient insurance information.
- Documentation. Maintain patient database including patient inquiries, patients awaiting clinic, patients awaiting treatment and treated patients. Document patient communications. Maintain patient medical records and documentation; perform records audits as required.
- Evaluation:
Evaluate patient wait times for treatment and overall patient waitlist, patient workflow and journey and recommends/implements improvements. Evaluate outcomes of care with the interdisciplinary team; suggesting process improvements and implementing team recommendations. - Customer Service. Utilize proactive problem-solving methods to handle difficult decisions regarding admissions and triage. Function as a liaison between program staff, Radiology, Movement Disorders Clinic, and Vendors regarding clinical and system issues as they relate to access and triage. Promote and maintain lines of communication, internal and external to the program. Respect patients by recognizing their rights; maintaining confidentiality. Report all adverse events and product complaints in accordance with established policies and procedures.
- Minimum of two years of RN experience
- Current IV placement experience
- Must be able to travel by Aerial Tram between Marquam Hill and CHH location
- BSN Graduates:
Baccalaureate Degree in Nursing from a program accredited by Commission of Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN) or Commission for Nursing Education Accreditation (CNEA) 30 days before start date. - ADN Graduates:
Associate Degree in Nursing from an accredited program 30 days before the start date. - Associate degree Nurses required to enroll in BSN program within 3 years of hire and complete within 5 years of hire
- Current, unencumbered Oregon State Registered Nurse License
- BLS from AHA required. New hires will be enrolled and required to complete during orientation.
- The RN is responsible for patient communication regarding pre-, intra, and post-imaging-related care/information.
