Position OverviewThe Triage Registered Nurse provides clinical triage, nursing assessment, patient education, care coordination, and perinatal nursing support in NARA NW's outpatient primary care clinics. This position serves as a key clinical resource for patients and the interdisciplinary care team by assessing patient needs, coordinating appropriate care, providing health education, and supporting quality patient outcomes. The Registered Nurse delivers culturally responsive, patient-centered care while collaborating with providers and other healthcare professionals to ensure safe, effective, and timely services.
Whatyou will do
- Provide clinical triage for patients through telephone calls, walk-in assessments, and nurse consultations, ensuring timely evaluation and appropriate disposition of patient needs.
- Coordinate prenatal and anticoagulation case management, including patient tracking, documentation, follow-up, and registry maintenance, as assigned.
- Maintain clinical registries and required program data in accordance with agency and regulatory requirements.
- Document nursing assessments, patient communications, and care provided in the electronic health record (EHR).
- Collaborate with providers, pharmacists, and interdisciplinary team members to coordinate patient care and address clinical needs.
- Assist with ordering and maintaining medical supplies and clinic medications as assigned.
- Participate in Quality Improvement (QI) initiatives and other agency activities.
- Provide cross-coverage and additional nursing support based on clinic and organizational needs.
- Provide nursing support, care coordination, patient education, and follow-up for patients enrolled in NARA Northwest's perinatal care program.
- Current, unrestricted Registered Nurse (RN) license in the State of Oregon required.
- Must obtain and maintain an unrestricted Registered Nurse (RN) license in the State of Washington within six (6) months of hire.
- Bachelor of Science in Nursing (BSN) required.
- Strong clinical assessment, critical thinking, and decision-making skills.
- Ability to provide patient-centered care and adapt to each patient's individual needs.
- Ability to work independently while collaborating effectively with providers, interdisciplinary teams, and community partners.
- Strong verbal and written communication skills with the ability to maintain confidentiality.
- Proficiency in the use of standard nursing equipment and electronic health record (EHR) systems.
