RN - Hospice

Position Overview

This position will be traveling primarily to Clark County Washington. The Hospice Nurse under physician direction completes medical requirements while coordinating a whole person plan of care. Position is accountable for direct and indirect quality care for patients of all ages. Care is according to the individualized needs of the patient, following the nursing process and in accordance with the Nurse Practice Act.

The position accepts professional development as an integral part of nursing practice. Coordinates patient care activities with other members of the health care team in a collegial relationship. Upholds the standards of confidentiality, attendance and punctuality.

• Travel primarily to Clark County Washington
• Complete medical requirements under physician direction
• Coordinate whole‑person plans of care
• Provide direct and indirect quality care for patients of all ages
• Follow the nursing process and Nurse Practice Act
• Coordinate patient care activities with other health care team members
• Uphold confidentiality, attendance, and punctuality standards

• Education to meet certification, license or registration requirements
• Oregon Registered Nurse License (upon hire)
• Washington Registered Nurse License (upon hire)
• National Provider BLS – American Heart Association (upon hire)
• At least 2 years medical/surgical experience in an acute care setting, or a combination of medical/surgical, long‑term care, home health, or hospice experience
• Experience using electronic medical record software
