Conviva Senior Primary Care seeks a dedicated LPN/LVN to provide professional nursing care in an on-site setting. You will work within an assigned unit or clinic and report directly to the Clinic Administrator at the Conviva Portland Care Center, 40 hours per week.
Ideal candidates are licensed LPN/LVN, with strong administrative skills and willingness to participate in Humana’s TB screening program; bilingual in English/Spanish and EMR/HEDIS experience are a plus.
