American Traveler is hiring a Nurse Navigator RN for an Orthopedic Foot & Ankle and Upper Extremity program, requiring at least 2 years of recent orthopedic OR or direct clinic patient care experience and an active OR RN license.

Requirements
  • Active, unencumbered Oregon RN license required
  • Active OR (Operating Room) RN license required
  • BLS certification from AHA required (traditional AHA only, RQI not accepted)
  • Minimum of 3 years overall nursing experience required
  • Minimum of 2 years recent orthopedic operating room or orthopedic direct clinic patient care nursing experience required
  • Epic charting experience required for consideration
  • Teaching hospital and Trauma Level I experience preferred
  • Nurse Navigator, Care Manager, or Telephone Triage/Advice experience preferred
  • Recent ambulatory orthopedic experience preferred
Responsibilities
  • Works within the Nurse Navigator unit supporting orthopedic Foot & Ankle and Upper Extremity faculty and interdisciplinary teams
  • Coordinates patient care from pre-consult through surgery and recovery, including pre- and post-operative journey tracking
  • Day shift schedule, 5x8-hour shifts from 7:00 AM to 3:30 PM
  • May meet with patients in clinic or on the inpatient unit as determined by the specialty team
  • Candidates may float within the hospital or to nearby affiliate hospitals per unit needs
  • Uses Epic charting system
  • Conducts comprehensive patient assessments to identify understanding of surgery, barriers to care, social determinants of health, learning style, and family support needs
  • Serves as the single point of contact coordinating pre-, intra-, and post-operative care as well as physical therapy for joint replacement patients
  • Tracks referral and outcome data to monitor volume, access, referral sources, and subsequent patient needs
  • Provides pre- and post-operative education and resources to patients, families, and caregivers
  • Acts as a clinical resource to the healthcare team on care standards and patient outcome goals
  • Works closely with care coordination and inpatient teams on admission and discharge planning
  • Collaborates with the Quality team to track key performance measures such as patient satisfaction, length of stay, and orthopedic-specific outcomes
  • COVID vaccination proof is required with no exemptions accepted
  • Local candidates are accepted at the same pay rate with no mileage radius restriction
  • Candidates must be willing to float to affiliate hospitals as needed; refusal to float may result in contract cancellation
  • Requested time off (RTO) must be submitted upfront and cannot exceed two weeks
  • Former OHSU employees or travelers must have a 6+ month gap before returning as a traveler
  • First-time travelers may be considered depending on position specifics
