Housecall Providers seeks an LPN to assist in planning, coordinating, and delivering hospice care under the supervision of a registered nurse. You will perform direct patient care, follow the plan of care, and collaborate with the interdisciplinary team to achieve positive outcomes.
The role requires a current Oregon LPN license, CPR, and at least 1 year of relevant experience. Schedule is Monday–Friday, full-time, with some after-hours or weekend backup shifts in the Tigard/Portland area.
