Position Overview Admissions RN Connected HH is seeking a full‑time, hourly Home Health Admissions RN to support our Portland Metropolitan service area. This role is ideal for nurses who excel in first‑visit assessments, care‑plan initiation, and establishing a strong foundation for each patient’s home‑health episode. This position follows standard business hours, Monday through Friday, providing predictable scheduling, strong interdisciplinary support, and a balanced workflow.

Key Responsibilities Admissions RN Responsibilities
  • Complete comprehensive Start‑of‑Care (SOC) nursing assessments in patient homes.
  • Initiate individualized care plans and establish expectations for the home‑health episode.
  • Conduct medication reconciliation, safety assessments, and disease‑specific education.
  • Coordinate care with interdisciplinary team members to ensure timely service initiation.
  • Provide skilled nursing interventions as needed during SOC visits.
  • Maintain accurate, timely documentation in accordance with regulatory and organizational standards.
  • Communicate effectively with physicians, caregivers, and internal team members.
  • Support safe transitions from hospitals, SNFs, and outpatient settings.

Schedule Business Hours Monday–Friday Normal business hours (daytime schedule) No evenings, nights, or rotating weekends Predictable workflow with full team availability during working hours

Qualifications RN Requirements
  • Current Oregon RN license (or ability to obtain).
  • One year of nursing experience preferred; home‑health or admissions experience a plus.
  • Strong assessment, communication, and critical‑thinking skills.
  • Ability to work independently in a field‑based role.
  • Reliable transportation and valid driver’s license.
