Latino Network is seeking a bilingual, mission‑driven Case Manager passionate about fostering the mental health and wellness of Latine communities by joining Zenit Centro de Serenidad, Latino Network’s culturally specific mental health clinic. They bring a deep commitment to equity and have experience working directly with youth and families, particularly in navigating access to critical social services such as rental assistance, utility support, food security, and healthcare.
Highly self‑motivated and well‑organized, the candidate thrives in a dynamic, collaborative team environment and demonstrates strong interpersonal skills that foster trust and meaningful relationships with community members. They are comfortable providing front desk support such as scheduling, engaging in outreach, case management, and advocacy work, and are eager to support the vision and mission of Latino Network’s Mental Health Program through culturally specific care and community engagement.
Description
Ability to meet the requirements for a Qualified Mental Health Associate as defined by the Oregon Administrative Rules and Mental Health and Addiction Certification Board of Oregon (MHACBO). The Case Manager position also requires Community Health Worker (CHW) certification or the ability to obtain certification within six months of hire. The Case Manager will primarily work in person at Zenit Centro de Serenidad and may also support community outreach efforts at locations including safety‑net clinics, shelters, libraries, DHS offices, health fairs, schools, home visits, and community events.
This role serves as one of the first points of contact for individuals seeking mental health services and plays a key role in reducing barriers to care. This position requires a bilingual, flexible, and self‑directed professional with strong organizational, communication, and problem‑solving skills. The Case Manager will provide front desk coverage, support clients in navigating healthcare and social service systems, assist with appointment coordination and follow‑up, provide health education and skill‑building support, refer clients to existing Latino Network programs, and maintain accurate documentation of services provided.
- Support recruitment, scheduling, outreach, and engagement strategies using culturally and developmentally appropriate communication.
- Serve as a welcoming first point of contact for individuals seeking services at Zenit Centro de Serenidad.

- Conduct in‑depth intakes through a culturally specific, trauma‑informed lens.
- Coordinate closely with the Clinical Supervisor and Clinic Coordinator to support smooth clinic operations.
- Maintain accurate and timely documentation of participant interactions and services in Reliatrax (and other required systems).
- Collect sensitive demographic information for ROADS submissions.
- Work closely with clinicians on case coordination to support client care.
