Case Manager Senior
This position exists to provide case management services for children and adults with developmental disabilities. The State Office of Seniors and People with Disabilities contracts with Multnomah County to provide case management services. The purpose of case management is to access, coordinate and assure the delivery of services and supports required by individuals with developmental disabilities. Case Management will assist eligible individuals under their individualized plan in gaming access to needed medical, social, educational, and other services.
Services include: (1) determination of appropriateness and need for developmental services; (2) evaluation of individual needs; (3) re-evaluation of individual needs; (4) development of an individual service plan; (5) assisting the individual in accessing needed services; (6) assisting the individual in accessing crisis services; (7) coordination of protective services; investigation and/or documentation of complaints; (8) coordination of services with other agencies who are involved with the individual (i.e., adult and family services, Children's Services Division, Vocational Rehabilitation Division, Senior Services Division, Social Security Administration, Veterans Administration, Department of Education, et cetera).
This is a Limited Duration position. This Limited Duration will end on 11/4/2027, but can not last more than two years and can end at any time.
As the Case Manager Senior, other duties will include:
- Provide comprehensive case management for youth and adults eligible for DDSD case management services who are identified as being at risk or exhibiting high risk behaviors.
- Support individuals in services with goals to improve quality of life and to maintain the highest possible ability to function within the community.
- Monitor services/supports via home visits, service provider visits and phone calls to ensure that they are receiving appropriate financial, medical, training, educational, transportation, and other identified/agreed to services/supports.
- Ensure resources are being appropriately used to meet clients' developmental disabilities needs.
- Develop and complete Individual Service Plans, which are completed at staffing and special meetings in client/families own homes as well as in provider settings of client needs due to developmental disabilities.
- Report possible protective services issues and provide initial information to assist the investigation.
- Prepare and submit routine and special reports. Assess level of functioning and level of support needed of the individual by completing the TXIX Waiver forms and submitting them to the State for approval.
- Review level of functioning and level of supports needed by updating the TXIX Waivers on an annual basis
- Notify the State when a client is out of a funded facility for any reason.
- Counsel and advise consumers/families in regard to placement and care/service plan choices, personal and emotional difficulties, financial planning, and interactional dynamics.
- Provide information and notice of client rights and coordinate resolution of grievances.
- Liaison with educational systems in order to assist child access to educational services that meet developmental disabilities needs.
- Review client files in both the CYAP and Adult units and provide analysis and direction to the IDD unit as requested.
- Give regular direction and technical assistance to Services Coordinators, DD Management and outside community members regarding behavior support standards, theories, rules, methods and resources.
