Somali Bilingual Case Manager 2

Multnomah County Department of County Human Services (DCHS), Aging, Disability and Veteran Services (ADVSD) is excited to announce we are currently seeking an experienced individual to provide services as a Somali bilingual Case Manager 2. As a Case Manager 2 you will work to maintain the health and welfare of older adults, and people with disabilities by arranging and monitoring long-term care services, as well as authorizing Medicaid, and Supplemental Nutrition Assistance Program (SNAP) benefits.
Key responsibilities
- Planning for and monitoring long-term services & supports (LTSS).
- Developing a comprehensive client-directed, outcome-based plan of care built on the client's needs assessment and in conjunction with the client and/or the client's representatives.
- Arrange for the provision of care in a variety of care settings and payment to care providers.
- Monitor the client's needs, risk status, and condition through home visits, telephone contact, and contact with care providers, contract registered nurses, and other collateral contacts.

Skills and Abilities
(KSA):
This position requires an advanced level in Somali (speak, read, write). Language testing is required.
Bilingual pay:
This position receives an additional 4% premium on all hours worked, because of the fluency in Somali language.
The eligible list created from this recruitment may be used to fill future regular, limited duration, temporary, on-call, full or part-time assignments within the department.

Minimum Qualifications/Transferable Skills*:
- Associate's degree in behavioral science, social science, or a closely related field or equivalent practical experience
- Three (3) years of case management work experience, working within the social or human services field.
- Must have a valid driver license by time of hire
- Must pass a criminal background check through the State of Oregon
- Must pass language proficiency test at an advanced level (speak, read, write)

- Ability to interpret/apply complex federal, state, and local laws, rules, regulations, policies and procedures related to program scope and operations
- Skill to research and analyze a variety of information and data; make recommendations based on research findings; prepare and write reports and documents; and present information
- Skill to perform general and/or risk/danger assessments, determine appropriate needs, create service plans, and utilize crisis intervention techniques as needed.
- Knowledge of case management methods, principles, techniques, and resources (including information/referral, assessment, interviewing, patient/client advocacy, risk intervention, resource utilization)
- Knowledge of available programs, public assistance providers, Medicaid, service agencies, protective services, referral centers, community residential resources, and crisis/emergency resources
