Position:
Part‑time Case Management Assistant (20 hr/week) – Aging, Disability and Veteran Services (ADVSD), Multnomah County, Oregon.
Pay Range: $26.86 – $33.04 hourly. Department:
Department of County Human Services (DCHS). Job type:
Regular. Status:
United States of America (Non‑Exempt).
- Assist clients with forms and screen for program eligibility.
- Search for potential placements and providers.
- Prepare files for transfers and assign to case managers.
- Collect client documents requested by case managers.
- Print, copy, fax, scan, and mail documents for case managers.
- Accompany clients to visit care facilities and medical appointments.
- Help clients obtain needed household items.
- Refer clients to community resources.
- Conduct home/field visits and transport clients.
- Use spreadsheets to track client and provider data.
- Participate in program outreach efforts and build relationships.
- High school diploma or GED.
- Two (2) years of general administrative experience, preferably in a social service setting, including direct client or public interaction.
- Pass a criminal background check.
- Valid driver’s license by the time of hire.
- Demonstrated ability to promote a culture of respect, inclusiveness, and appreciation of diverse perspectives.
- Knowledge of cultures, customs, and social experiences of communities experiencing marginalization and barriers to opportunity.
- Experience working closely with consumers in their home.
- Decision-making skills in coordinating moves or shopping.
- Organizational and prioritization skills to meet deadlines, working both independently and as part of a team.
- Clear and concise verbal and written communication skills.
- Ability to assess and refer clients to proper agencies or service resources.
- Understanding and application of procedures, requirements, regulations, and policies for social services programs.
- Good judgment, diplomacy, and tact when dealing with sensitive, complex, or confidential issues.
- Advanced computer and office equipment proficiency.
- Part‑time, 20 hours per week.
- Union‑represented part‑time (20 hours a week) position eligible for overtime.
- Schedule:
Monday – Friday, 8:00AM – 5:00PM (core business hours). - Location:
11982 NE Glisan St., Portland, OR 97220 (Mid County Branch).
