  • Get people housed
    • Outreach, assessments, and connecting clients to SPC and other housing options
    • Navigate applications, documentation, and barriers

    • Coordinate placements, inspections, deposits, and lease signings
    • Build strong relationships with landlords and partners
  • Keep them housed
    • Ongoing case management, advocacy, and eviction prevention
    • Support renewals, moves, and long‑term stability

Location & Schedule

Location: Golden West (707 NW Everett St. Portland, OR 97209)

Schedule: Monday – Friday 8:00 am‑5:00 pm (Saturday/Sunday Off)

Compensation
  • QMHA‑R: $25.05 – $33.24 /hr
  • QMHA‑1: $27.62 – $36.65 /hr

* Final compensation will be determined based on relevant experience, education, credentials/licensure, and internal equity. External candidates:
Offer max for QMHA‑R $29.07 /hr / QMHA‑1 $ 32.05 /hr per our Union contract. Commensurate with relevant experience.

Minimum Qualifications
  • QMHA‑1 or QMHA‑R required.
  • Must be registered with MHACBO to meet at least QMHA‑R requirements.
  • Bachelor's degree in behavioral sciences or a combination of at least three years of work, education, training, or experience in healthcare, chemical dependency, mental health treatment, or social services.
  • Must possess a current driver's license, access to a vehicle, and qualify as an Acceptable Driver as designated in Central City Concern’s Fleet Safety policy. Must pass initial drivers training within 60 days of being an approved driver and continue recertification training. Must maintain vehicle insurance coverage of a minimum of $100,000/$300,000 personal auto liability coverage.
  • Must meet CCC privileging requirements as required by FTCA.
  • Must have current CPR certification prior to start date.
  • Must pass a pre‑employment drug screen, TB test, and background check. Includes clearance by the DHS Background Check Unit.
  • Bi‑lingual in Spanish preferred.
  • Ability to complete forms thoroughly, accurately, and with legible handwriting.
  • High tolerance for ambiguity and change.
  • Ability to adhere to Central City Concern’s drug‑free workplace and maintain compliance with the Drug Free Workplace Act of 1988.
  • Physical ability to bend, stoop, kneel, squat, twist, reach, pull, and lift heavy objects; able to climb stairs several times a day.
  • Occasional lifting of items up to approximately 25 pounds.
  • Must carry an agency cell phone for work use; phone will be provided by Central City Concern.
  • Adherence to agency’s non‑discrimination policies.
  • Ability to interact effectively with co‑workers and clients from diverse backgrounds, including differences in ethnic, religious, cultural, and sexual orientations.
Essential Duties
  • Conduct regular community outreach, including street outreach, to identify and locate eligible participants.
  • Participate in regular team meetings and huddles.
  • Schedule eligible clients for housing assessments, including MSST, and connect them to appropriate medical and behavioral health programs.
  • Determine housing eligibility and coordinate placement in emergency shelter or transitional housing, or identify alternative housing options based on client needs and preferences.
  • Enter and maintain accurate client data in HMIS.
  • Coordinate with emergency shelter or transitional housing team members to support client placements and progress.
  • Support clients in maintaining temporary placements through behavioral interventions, advocacy, and participation in care conferences.
  • Engage regularly with clients to build housing readiness and independent living skills.
  • Assist clients in achieving housing readiness, resolving legal issues, and gathering required documentation.
