General ResponsibilitiesThe In-Reach Housing Case Manager is responsible for facilitating housing placements, promoting housing sustainability, and supporting participant engagement and socialization within the community for individuals placed through our partner shelter agencies.
This role centers on securing stable housing, fostering sustainability, and actively engaging participants to support their transition toward self-sufficiency as they prepare to exit housing subsidies. The ideal candidate is a creative problem-solver who excels at building rapport, staying organized, and motivating others. They will use assertive engagement strategies to help participants remain focused and make progress toward their housing stability goals.
SpecificResponsibilities
Access, Housing, and Retention:
- Assist in the development and implementation of individual goals and objectives to attain stability, housing and ultimately maintain self- sufficiency.
- Address barriers to individual goals and objectives including (but not limited to) - mental health, substance use, income development, tenant education, socialization, basic needs.
- Identify, locate and coordinate delivery of services for participants.
- Meet with participants as needed to provide and coordinate delivery of services.
- Coordinate with agency staff to provide consistent quality and comprehensive services to participants.
- Develop and maintain relations with community resources to assure service availability to participants.
- Create and maintain relationships and resource knowledge specific to the population served.
- Coordinate housing placement and retention services.
- Conduct home visits (this duty requires the ability to operate the agency automobile).
- Attend staff meetings, case management meetings and partner meetings as assigned.
- Maintain accurate, updated and comprehensive participant files.
- Maintain accurate and updated documentation of services in HMIS.
- Adhere to allocated budgetary guidelines.
- Provide monthly Case Management Reports as requested.
- Facilitate groups such as Retention, Men’s and Women’s Engagement, etc.
- Follow Case Management Best Practices.
- Other duties as assigned.
- Acquire First Aid/CPR/AED certification within 30 days of hire and be retested biannually thereafter throughout employment.
- Complete Bloodborne Pathogens and Narcan Training within 30 days of hire and be retested annually thereafter throughout employment.
- Demonstrate reliability by being present for work as scheduled, arriving and leaving on time and taking breaks in expected time frames.
- Interact effectively with persons of diverse ethnic backgrounds, religious views, cultural backgrounds, and sexual orientations and treat each individual with respect and dignity.
- Attend agency and program staff meetings.
- Hearing and vision adequate for interaction with participants, interaction with staff and interaction with the general public.
- Hearing adequate for telephone work. Vision adequate for close work.
- Hand and finger dexterity adequate to operate standard office equipment.
- Ability to walk, bend, stand, sit, reach, stoop, pull, squat, and climb stairs.
- Sitting at a desk completing paperwork and working on a computer for lengthy periods.
- Mobility to allow responsive traveling to participant residences located around the metro area.
- Ability to perform CPR and other emergency life-saving care with the assistance of teammates until emergency responders arrive.
