Central City Concern is seeking a Case Manager to support residents in the Hazel Heights program in Portland, helping clients access treatment, housing stability, and self-sufficiency while maintaining professional boundaries.
Schedule:
Monday–Friday, 9:00am–5:30pm; weekends off; located at Hazel Heights, Portland, OR.
Responsibilities include coordinating with Housing and Health Services, referring clients to CCC and external agencies, and entering data into HMIS; compliance with privacy rules
