Case Manager - Hazel Heights

The Case Manager acts to ensure the stability of individuals within the program properties and will be familiar with and able to access resources for clients, including alcohol and drug treatment, mental health treatment and any other identified needs. The Case Manager must have a good understanding of dual relationships, professional boundaries, and appropriate behavior. The candidate will have the skill s needed to support and assist clients in improving and maintaini ng recovery, retaining housing, and achieving self-sufficiency.

Monday - Friday, 9:00am - 5:30pm (Saturday & Sunday Off)

$23.30 - $30.91/hr

Final comp will be determined based on relevant experience, education, credentials/licensure, and internal equity. External candidates:
Offer max is $27.03/hr per our Union contract.

• High School DiplomaorGED andsix months of experience working with the homeless population, behavioral health environment, human services, or related environment; OR 6 months experience in a high-volume customer service role.
• Must pass a pre-employment drug screenand background check.
• Will be required to carry an agency cell phone for work use. Cell phones will be provided by Central City Concern.

• Valid driver's license preferred. If required to drive,mustpass a DMV background check, and bedesignatedan  acceptable  driver as outlined in Central City Concern's Fleet Safety Policy. • Understanding of recovery terminology and ability to apply the concepts highly preferred.

• Work closely withthe Supportive Housing and Health Servicesteamensuringasafe and healthy recovering environment.
• Build understanding of and appreciation for diversity amongresidentsand support their connection to culturally responsive and culturally specific services as requested.
• Process resident/client intakes and discharges with completion of associated forms.
• Maintain resident/client files including progress notes and other records.
• Meet bi-weekly with clientsalong with other referral sources as needed.
• Refer clients to Employment Access Centeralong with otherCCC (Central City Concern) and outside agencies as appropriate.
• Identify community and individual issues which could result in a change in a residents/client’sstatus and work to achieve appropriate resolution.
• Connect residents/clientsto appropriateself-sufficiency and community-based resources.
• Network withvariouscommunity providers.
