Central City Concern (CCC) is seeking a Housing Case Manager (QMHA) to help chronically homeless adults move from streets to stable housing through Shelter Plus Care. You will conduct outreach, assess participants, coordinate housing placements, and support ongoing stability within a housing-first framework.
The role requires QMHA certification, a driver’s license, CPR, and ability to navigate complex documentation and barriers. Bilingual Spanish is preferred.
