Personal Agent, Case Manager

• Assist customers (and their families) to navigate the support services system, including using person-centered planning techniques to assess their needs and develop plans and goals;
• Secure resources and services (in areas such as employment, housing, crisis, and direct support providers);
• Address health and safety needs.
• Advocate with and on behalf of people supported, offering care-coordinated problem-solving skills for the caseload.
• Manage intensive paperwork including plan development, billing, daily & weekly progress notes, caseload files, and correspondence.
• Monitor the quality of services, ensuring choice and independence.

• Bachelor’s degree in a behavioral science, social science, or closely related field; or, a Bachelor’s degree in any field and one year of human service related experience; or, an Associate’s degree in a behavioral science, social science, or closely related field and two years of human service related experience; or, a high school diploma or GED and three years of human services related experience.
• Able to drive on the job, which means that you are at least 18 years old, have a current driver’s license, a reliable personal vehicle, at least two years of driving experience, auto insurance, and a clean driving record.
• Must pass a reference check and criminal history background check.
• Demonstrated commitment to customer self-determination and personalized support.
• Demonstrated ability to prioritize, meet deadlines and manage multiple projects in a fast-paced environment.

Demonstrates expertise in person-centered planning, resource navigation, and advocacy for individuals in need of support services. Proficient in managing documentation and ensuring quality service delivery while promoting customer self-determination.
