The LPN/LVN provides professional nursing care to patients within an assigned unit, clinic or region.
As an LPN/LVN you will perform clinical tasks such as:
* You will assess patient health problems and needs, take vital signs, administers medication, develop and implement nursing care plans, and maintain medical records.
* You may advise patients on health maintenance and disease prevention or provide case management.
* You understand own work area professional concepts/standards, regulations, strategies and operating standards.
* You work within defined parameters to identify work expectations and quality standards, but have some latitude over prioritization/timing, and work under minimal direction.
Required Qualifications
* Current unrestricted licensure as an LPN or LVN in the state of Texas
* Excellent customer service
* Strong clinical background
* Strong sense of engagement to be a team player
* Must be passionate about contributing to an organization focused on improving consumer experiences
Preferred Qualifications
* Bilingual in English/Spanish with the ability to read, write and speak in both languages
* Hands-on professional
* Phlebotomy experience
* Experience with Electronic Medical Records
* Experience with HEDIS
* Knowledge of primary care medicine requirements (state specific).
* Working knowledge of clinical operations
Work Hours:
Monday - Friday 8:00 to 5:00

Onsite at Center Well Senior Primary Care office located at 107 Cedar Drive, Portland, TX 78374
This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Pay Range
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$48,900 - $66,200 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
