The LPN/LVN provides professional nursing care to patients within an assigned unit, clinic or region.
Responsibilities- Assess patient health problems and needs, take vital signs, administer medication, develop and implement nursing care plans, and maintain medical records.
- Advise patients on health maintenance and disease prevention or provide case management.
- Understand own work area professional concepts, standards, regulations, strategies and operating standards.
- Work within defined parameters to identify work expectations and quality standards, with some latitude over prioritization/timing, and work under minimal direction.
- Perform clinical tasks and support the organization’s patient‑facing role within Humana’s Tuberculosis screening program, including required TB screening.
- Current unrestricted licensure as an LPN or LVN in the state of Texas
- Excellent customer service skills
- Strong clinical background
- Strong sense of engagement to be a team player
- Must be passionate about contributing to an organization focused on improving consumer experiences
- Bilingual in English/Spanish with the ability to read, write and speak in both languages
- Hands‑on professional experience
- Phlebotomy experience
- Experience with Electronic Medical Records
- Experience with HEDIS
- Knowledge of primary care medicine requirements (state specific)
- Working knowledge of clinical operations
Work Hours: Monday – Friday 8:00 to 5:00
Location: Onsite at Center Well Senior Primary Care office located at 107 Cedar Drive, Portland, TX 78374.
Eligibility: This role is considered patient facing and is part of Humana's Tuberculosis screening program. If selected for this role, you will be required to be screened for TB.
Pay Range: $48,900 - $66,200 per year
