Travel Nurse Source  is partnering with Adelphi Healthcare to place a qualified Hospice RN in Portland, TX. This is a part‑time freelance contract position, starting ASAP.

• Active Registered Nurse (RN) license
• Graduate of an accredited nursing program (ADN or BSN)
• Minimum of 1 year of RN experience; hospice, palliative care, oncology, or med‑surg experience preferred
• Strong assessment, critical thinking, and communication skills
• Ability to provide compassionate care in emotionally challenging situations
• Ability to work independently and as part of a multidisciplinary team

• Assess patient physical, emotional, and psychosocial needs
• Develop, implement, and evaluate individualized care plans
• Manage pain and symptoms in accordance with physician orders and care plans
• Administer medications, treatments, and interventions safely and accurately
• Provide education and support to patients and families regarding disease progression, care plans, and end‑of‑life needs
• Coordinate care with physicians, social workers, chaplains, aides, and other team members
• Monitor patient status and report changes promptly
  
  Document assessments, care, and patient responses accurately and timely
• Participate in interdisciplinary team meetings and care planning
• Provide emotional support and guidance to patients and families during end‑of‑life care
