Key Responsibilities
  • :

    Home Visits:
    Conduct home visits to assess and create personalized care plans for clients and their caregivers
  • Training & Support: Train and support caregivers, ensuring they can meet their loved ones' needs
  • Reporting & Coordination: Monitor and report on clients’ health, coordinating with their primary care physicians, community resources
  • Ongoing Care: Facilitate smooth transitions and ongoing care for clients
  • Compliance: Maintain compliance with all relevant regulations and standards
  • Community Awareness: Participate in community events to spread awareness and enroll new caregivers
  • Role Model: Embody Entyre Care’s mission, values, and culture, acting as a role model for the team
What You Will Brin
  • Bachelor's degree required, must be LSW or LISW, human services, social work, or a related fiel
  • At least one year of experience working with elders or adults with disabilitie
  • Valid driver's license and automobile insurance, as travel within Ohio is require
  • Demonstrated experience in case management, care planning, and assessment in a healthcare settin
  • Outstanding proficiency in common software and web-based applications including Microsoft Excel, Word, PowerPoint, and Team
  • Master's degree in social work or a related field preferre
  • Experience working in Structure Family Caregiving (SFC) or a similar setting preferre
  • Certifications in gerontology or case management preferre
