General Summary
Works under the supervision of the Nurse Manager or Assistant Nurse Manager. The RN Admission Nurse evaluates and admits patients for all home care disciplines and makes regularly scheduled visits as needed. Duties include admission; revisit; OASIS data collection; developing the plan of care; referral of ancillary services (rehabilitation therapists, social worker, home health aide) and completion of required documentation. Assists with QAPI programs.
Performs other duties as assigned.
- Education:
High School Diploma or successful completion of an equivalent high school exam required. - Graduate from an accredited school of nursing required.
- BSN preferred.
- Licensed to practice as an RN in Ohio required.
- Licensed to practice as an RN in Kentucky to be obtained after hire required.
- BLS within 90 days of hire required.
- Prior home care experience preferred.
Specific Duties and Performance Expectations
Provides to the patient/family information regarding the patient’s rights to formulate Advance Directives. Instructs the patient/family in processes to follow if assistance is needed after regular office hours. Reviews general emergency preparedness instructions with the patient/family.
