Gentiva's Heartland Hospice is seeking a compassionate Hospice LPN to provide direct bedside care in patients’ homes, assisted living, or facilities. You will work under RN supervision to implement individualized plans of care and support families during difficult times.
The role requires an accredited Practical Nursing program graduate with an active LPN/LVN license, CPR, and 2–5 years hospice/home health experience.
