POSITION TITLE: NURSE (LPN)
REPORTS TO: BUILDING PRINCIPAL
OVERTIME STATUS: NON EXEMPT
PAY GRADE: 116

JOB SUMMARY: Performs intermediate professional work ensuring the health of students in schools; does related work as required. Work is performed under the regular supervision of the Principal and Supervisor of Health Services.

• Provides nursing care to students; implements grade level screening program for students; maintains and operates a school clinic; prepares and maintains student and clinic records.
• Provides health care services for students in school system.
• Performs nursing assessment of each student sent to school health clinic.
• Makes observations and assesses condition; implements appropriate plan of care for each student examined.
• After observation and assessment, counsels with student, parents and/or teachers about problems observed.
• Administers medications and/or treatments; administers first-aid.
• Screens students for hearing, vision, height, weight, and blood pressure at appropriate grade levels.
• Refers students for outside medical care, as needed.
• Logs information pertaining to student/staff clinic visits and medical care given.
• Provides instruction on nutrition, hygiene, grooming, weight control, first aid, disease prevention, substance abuse and other related health concerns; provides health educational materials and information.
• Provides health counseling to students, parents and school personnel.
• Maintains records of medication administration as well as treatments and procedures performed.
• Reviews student immunization records on all students to ensure that each student is in compliance with state immunization laws for school attendance; maintains immunization records on all students.
• Provides immunizations to students and staff of Portsmouth Public Schools, as directed by the School Physician.
• Maintains a neat, attractive clinic; maintains adequate clinic supplies.
• Performs related tasks as required.

Thorough knowledge of common medical practices, procedures and techniques associated with a standard physical examination; thorough knowledge of the preliminary recognition of medical problems and the administration of medications related to treatment and first aid; ability to administer medications and apply therapeutic treatments as required; ability to follow technical oral and written instructions in exact detail; ability to maintain accurate records.

This is light work requiring the exertion of up to 20 pounds of force occasionally, up to 10 pounds of force frequently, and a negligible amount of force constantly to move objects; work requires reaching, standing, walking, and fingering; vocal communication is required for expressing or exchanging ideas by means of the spoken word; hearing is required to perceive information at normal spoken word levels;

visual acuity is required for color perception, preparing and analyzing written or computer data, use of measuring devices, determining the accuracy and thoroughness of work, and observing general surroundings and activities; the worker is subject to inside environmental conditions, hazards, and atmospheric conditions.

License or Certificate in Practical Nursing as currently defined by state law.
