Home Health RN Case Manager

Position Requirements :
  • Minimal

    Education:

    1. Graduate of accredited school of nursing.
    2. Bachelor’s degree of Science in Nursing is preferred.
  • Minimal

    Experience:

    1. Minimum of one (1) year nursing experience. 2. One (1) year skilled nursing facility experience preferred.
  • Other

    Qualifications:

    1. Current Nursing License. 2. CPR certified.
    3. Ability to function with minimal supervision.
    4. Excellent communication, organizational and human relation skills.
Essential Duties and Responsibilities:
  • Follows established Standard of Nursing Practices and implement Facility policies and procedures.
  • Knowledgeably and competently performs nursing skills.
  • Identifies safety hazards and initiates corrective action immediately including assuring the safety of Residents under his/her supervision.
  • Interpret existing Policies and Procedures to Nursing Assistants, Restorative Nursing Assistants, Residents, Families and Physicians.
  • Upon admission and on an ongoing basis, assesses the patient’s physical and emotional status, level of competency/knowledge, and safety.
  • Identifies health and safety hazards and initiates corrective action immediately including assuring the safety of Residents under his/her supervision.
  • Supervises and evaluates all direct resident care and initiates corrective action as necessary.
  • Receives and provides report from/to nurse he/she is relieving/coming on duty and records sufficient information to implement appropriate follow-up as necessary.
  • Develops an accurate comprehensive care plan based on each resident's assessment.
  • Provides direct resident care and gives instruction to Nursing Assistants.
  • Consistently works cooperatively with residents, nursing staff, physicians, families, consultant personnel, community agencies; and ancillary service providers.
  • Formulates and periodically updates care plan as necessary and as required by Regulations.
  • Listens to resident, family or physician concerns and initiates corrective action and/or reports concerns to RN Supervisor.
  • Provides clinical supervision of Nursing Assistants providing direct resident care.
  • Assesses resident's needs and initiates development of individualized plans of resident care.
  • Participates in orientation of new employees to facility policies and procedures and processes.
  • Participates in interdisciplinary case conference and discharge planning.
  • Records and reports changes in the patient’s condition to physician and RN Supervisor in a timely manner.
  • Documents all orders in accordance with regulatory and facility requirements.
  • Delivers care according to physician orders, plan of care and facility standards.
  • Orders, prepares and administers medications, enteral and parenteral nutrition and treatments as ordered.
  • Maintains current technical and clinical skills, certifications and licensing.
  • Attends and participates in in-services, clinical record review process, conferences, meetings and facility Continuous Quality Improvement activities.
  • Performs other duties as assigned by supervisor or manager.
  • Follows standard precautions and infection control procedures.
  • Lifts residents and transports during fire/disaster drills and in actual emergencies. May respond to emergencies outside unit or campus as needed.
  • Maintains strict confidentiality of resident, employee and business information.
  • Provides for a safe environment for residents, physicians and employees.
  • Meets with Residents and residents’ families/representatives to resolve resident concerns.
  • Participates in orientation of new employees and evaluations of CNAs and LPNs.
  • Performs other duties as assigned by supervisor.
