Position Summary
The Registered Nurse (RN) collaborates with all members of the care team to provide world‑class care to patients and staff at Martin’s Point. The RN coordinates delivery of primary and specialty services, ensuring high quality care while utilizing resources efficiently. The RN provides independent assessments, patient education, and proactive problem‑solving to enhance patient satisfaction and provider efficiency.
Key Outcomes & Responsibilities- Coordinate with internal providers, external specialty providers, community health resources, urgent care centers, and hospitals to maintain continuity of patient care.
- Prioritize and schedule specialty/primary care services, maximizing available resources and managing walk‑in visits and clinical triage.
- Perform patient assessments to determine urgency of services using standard protocols for disposition.
- Maintain adequate inventory of medical supplies, instruments, and equipment meeting quality and infection control standards.
- Provide patient education on diagnoses, lab results, procedures, medications, risk factor modifications, exercise, and follow‑up activities via phone, portal, or face‑to‑face.
- Identify patients with gaps in care, strategize appropriate actions to facilitate closure, and collaborate with providers, clinical support staff, and population health nurses.
- Review scheduled appointments to identify chronic disease patients and analyze records for gaps in care.
- Participate in daily huddles and team meetings to improve workflows and patient outcomes.
- Receive clinical direction from on‑site RN leaders, quality specialists, supervisors, or managers.
- Triage telephone calls, applying standard protocols for disposition (schedule appointment, refer to urgent/emergency care, or provide home advice).
- Communicate with patients via person, telephone, or portal to assess needs and coordinate appropriate care.
- Support health maintenance, wellness promotion, and disease management by assisting providers with patient‑level data and leading flu clinics and community wellness initiatives.
- Process documents (prescription refills, DME orders, forms, lab results, notes) per protocol and guidelines.
- Develop, implement, and follow up on nursing plans of care customized to patient demographics, culture, resources, barriers, and readiness for adherence.
- Conduct nursing care visits for injections, nebulizer treatments, chronic disease visits, coagulation therapy, HIV counseling, wound care, blood pressure monitoring, spirometry, peak flow, medication counseling, and other appropriate interventions.
- Perform point‑of‑care testing (rapid COVID, flu, strep, RSV, blood glucose, urinalysis, A1C, pregnancy testing, urine drug screens) and random pill counts.
- Utilize appropriate PPE per protocol.
- Associate degree in nursing; bachelor’s degree preferred.
- Minimum two years of nursing experience, including at least one year responsible for independent decisions (e.g., inpatient medical/surgical, triage, home health, clinical specialist).
- Current, unrestricted Registered Nurse licensure in Maine.
- BLS (Basic Life Support) required.
- ACLS (Advanced Cardiac Life Support) required for cardiac nurse roles.

- Research and identify innovative solutions to patient, resource, or coordination challenges.
- Proactive clarification and resolution of process issues.
- Accurate, complete documentation.
- Development of policies, procedures, and training on equipment.
- Act as a specialty resource to peers.
