Case Manager - PT

Responsibilities

• Being assigned youth before their release from custody and remaining with them through the entire reentry process.
• Developing individual plans of care and delivering a wide array of support services to youth including educational, vocational and employment opportunities.
• Preparing records of client visits, progress, and case notes.
• Responding to emergencies and crisis interventions as needed.
• Establishing working relationships with families, court personnel, referral agencies and other service providers.
• Traveling in own vehicle and having mileage reimbursed.
• Working a flexible schedule including weekends, weekday evenings and daytime hours.

• Bachelor's degree from an accredited college or university in human services, criminal justice or a related field.
• 1-3 years of case management experience (paid, volunteer, internship or practicum).
