- Complete initial, comprehensive, and ongoing assessments of patients and families to determine home health or hospice needs.
- Provide respectful, compassionate bedside care in patients' homes.
- Develop patient-focused care plans that establish goals based on nursing diagnoses and incorporate rehabilitative nursing actions.
- Collaborate with interdisciplinary team members.
- Active nursing certification (state‑licensed).
- Valid driver’s license, reliable transportation, and current auto insurance.
- Strong communication skills and a team‑oriented mindset.
- Ability to work independently in home settings.
- Commitment to patient‑focused care.
- Current CPR Certification.
