As a nurse with Elite, you play a vital role in “Keeping Our Community Home,” fostering meaningful connections, and ensuring patients receive the best possible care.

• Complete initial, comprehensive, and ongoing assessments of patients and families to determine home health or hospice needs
• Provide respectful, compassionate care in patients’ homes
• Develop patient‑focused care plans establishing goals based on nursing diagnoses and incorporating rehabilitative nursing actions
• Collaborate with interdisciplinary team members

• Active nursing certification (state‑licensed)
• Valid driver’s license, reliable transportation, and current auto insurance
• Strong communication skills and a team‑oriented mindset
• Ability to work independently in home settings
• Commitment to patient‑focused care
• Current CPR certification
