What You Will Do

• Accepts assignments of clients from the R.N. Case Manager, provides nursing care through the utilization of the home assessment, nursing diagnoses, and physician-approved care plans. Performs treatment(s) ordered by the physician and delegated by the R.N. Case Manager.
• Observes signs and symptoms and reports them to the R.N. Case Manager.
• Notes changes in physical or emotional status of clients.
• Promotes client and caregiver independence in self-care.
• Utilizes electronic documentation of client care; complies with Agency policies regarding timeliness in submission of documentation and synchronization of data.
• Follows Agency policies regarding universal precautions, and safety guidelines.
• Fulfills other assignments as requested by the R.N. Case Manager, VP of Clinical Services, or Director of Professional Services.

• Holds a current Licensed Practical Nurse license in Virginia.
• Has a minimum of one year of nursing experience.
• Has current CPR certification.
• Has excellent communication, organizational, and human relations skills.
• Is able to function with minimal supervision.
• Is able to manage multiple projects/changing priorities.
• Is able to employ critical thinking skills.
