Self‑Sufficiency Case Manager What You'll Do:

Empower & Support:
Provide case management services that help individuals and families set and achieve personal goals.
Guide Participants:
Conduct assessments, develop individualized plans, and assist in stabilizing housing situations.
Manage Cases:
Keep detailed records, document all contacts, and maintain participant confidentiality.
Promote Self‑Sufficiency:
Support participants in becoming independent from public assistance.
Community Engagement:
Build relationships with service providers, attend networking meetings, and advocate for participants.
Stay Organized:
Ensure accurate documentation, track referrals, and complete necessary reports on time.

Strong communicator, both written and verbal, who easily connects with people.
Can assess needs and create action plans that drive real results.
Experience working with multiple population types and advocating for individuals.
Problem‑solving and conflict resolution, handled with confidence.

Education and Experience:

Minimum of two (2) to four (4) years of education and/or experience in a related field required.
A four‑year post‑secondary degree in Human Services preferred.
