Self‑Sufficiency Case Manager What You'll Do:
- Empower & Support:
Provide case management services that help individuals and families set and achieve personal goals. - Guide Participants:
Conduct assessments, develop individualized plans, and assist in stabilizing housing situations. - Manage Cases:
Keep detailed records, document all contacts, and maintain participant confidentiality. - Promote Self‑Sufficiency:
Support participants in becoming independent from public assistance. - Community Engagement:
Build relationships with service providers, attend networking meetings, and advocate for participants. - Stay Organized:
Ensure accurate documentation, track referrals, and complete necessary reports on time.
- Strong communicator, both written and verbal, who easily connects with people.
- Can assess needs and create action plans that drive real results.
- Experience working with multiple population types and advocating for individuals.
- Problem‑solving and conflict resolution, handled with confidence.
- Team player who also thrives when working independently.
- Approach cultural and economic differences with sensitivity and understanding.
- Minimum of two (2) to four (4) years of education and/or experience in a related field required.
- A four‑year post‑secondary degree in Human Services preferred.
- Starting salary of $20.44 to $31.34 per hour + benefits.
- Full‑time (40 hours per week), 12‑month year.
