As a Licensed Practical Nurse (LPN) with Montcare Potomac, you will play a vital role in providing high-quality, patient-centered care to our residents in our rehabilitation and skilled nursing centers. You will work closely with our interdisciplinary team to develop and implement individualized care plans, administer medications, and provide routine nursing care to our patients. Your exceptional communication skills and ability to work effectively in a fast-paced environment will enable you to build strong relationships with our patients, families, and healthcare colleagues.

• Provide direct patient care, including administering medications, wound care, and vital sign monitoring
• Develop and implement individualized care plans in collaboration with the healthcare team
• Contribute to a safe and clean environment by adhering to infection control protocols and maintaining order and organization in the patient care areas
• Communicate effectively with patients, families, and healthcare colleagues to ensure seamless care transitions and address patient concerns
• Participate in patient education and support to promote healthy habits and disease prevention
• Document patient care and progress accurately and timely

• Currently licensed as a Licensed Practical Nurse (LPN) in the state of Maryland
• 0 years of experience in a healthcare setting (new graduates welcome!)
• Strong communication and interpersonal skills
• Ability to work effectively in a fast-paced environment
