Overview
St. Luke's University Health Network is seeking a Full‑Time Registered Nurse ACT to join a multidisciplinary Assertive Community Treatment Team. The role involves delivering nursing care in the community to support symptom management and coordinate medical and psychiatric care for individuals in the ACT program.
Responsibilities- Meet with ACT participants in their homes or community venues to provide in‑vivo services that improve quality of life and maintain community tenure.
- Assist with activities of daily living, budget planning, housing stability, benefit access, meal planning, transportation, and side‑by‑side coaching and training.
- Transport and accompany clients to community activities or appointments.
- Provide medication services, including delivery and observation of ingestion, as directed by the team psychiatrist, nursing staff, and Team Leader.
- Coordinate services for an assigned group of clients, including developing, writing, implementing, evaluating, and revising treatment goals.
- Offer individual and supportive counseling to manage symptoms and provide crisis response on a rotating schedule, including primary and backup on‑call duties.
- Document all services promptly in the electronic health record in accordance with program and network standards.
- Work weekend, evening, and holiday shifts on a rotating schedule to cover program needs.
- Cross‑train with other ACT specialties to provide services such as housing or vocational support.
- Under direction of the ACT psychiatrists and leaders, package medications for delivery, attend medical appointments with participants, coordinate with medical providers, and maintain accurate medication documentation.
- Conduct health assessments upon admission and maintain annual reviews for all participants.
- Arrange required lab work per network protocol and evaluate medication effectiveness, complications, and side effects.
- Provide educational groups on health-related topics such as exercise, nutrition, and medication management.
- Perform other duties as assigned by the ACT leadership.
- Registered Nurse with a current license to practice in Pennsylvania.
- Experience in psychiatric nursing, work with individuals with mental illness, and exposure to forensic or co‑occurring substance use disorders (preferred).
- Completed Basic Life Support (BLS) and CPR certification.
- Valid driver’s license with a clean driving record in compliance with St. Luke’s policies.
- Ability to perform heavy work including lifting up to 50 lbs., and sit, stand, and walk for at least 6 hours per day.
- Good visual and auditory acuity and overall physical and mental fitness.
Heavy exertional activity, including lifting or carrying up to 50 lbs. and sustained standing or walking for at least 6 hours in an 8‑hour shift. Frequent stooping, crouching, grasping, and fine manipulation are required. Visual and auditory acuity is necessary to deliver comprehensive care.
Education and TrainingRegistered Nurse licensure in Pennsylvania. BLS and CPR certification. Experience in psychiatric nursing preferred.
