Responsibilities:
- Provide high-quality patient care, demonstrating a strong understanding of evidence-based practices and patient-centered care
- Collaborate with interdisciplinary teams to develop and implement individualized care plans
- Monitor patient outcomes, identifying areas for improvement and implementing changes as needed
- Participate in quality improvement initiatives and contribute to the development of policies and procedures
- Communicate effectively with patients, families, and healthcare teams to ensure seamless transitions and optimal care
- Mentor and educate nursing staff, students, and other healthcare professionals
- Perform duties in accordance with organization's mission, values, and standards of care
