ACT/FACT Housing Specialist
Member of the multidisciplinary Assertive Community Treatment (ACT) Team, the Housing Specialist provides supportive services—including outreach, monitoring, assessment, service planning, and support network building—to help participants achieve stable community living.
Key Responsibilities- Meet with ACT participants in homes and community venues to deliver in‑vivo services that enhance quality of life and support community tenure. Services encompass: assistance with activities of daily living; budget planning and money management; stable housing maintenance; accessing benefits and entitlements to increase financial independence; meal planning and grocery shopping; reliable transportation; side‑by‑side coaching; and transport to scheduled community activities or appointments.
- Provide medication services, including delivery and observation of ingestion, as directed by the team psychiatrist, nursing staff, and Team Leader.
- Collaborate with ACT staff to coordinate services for an assigned client group, developing, writing, implementing, evaluating, and revising treatment goals while offering individual and supportive counseling for symptom management.
- Deliver on‑call crisis services on a rotating schedule, covering both primary and backup responsibilities as directed by the Team Leader.
- Complete timely documentation of all services in the electronic health record in accordance with program and network standards.
- Work weekend, evening, and holiday shifts on a rotating schedule as directed by the Team Leader.
- Cross‑train with other ACT specialties to provide additional services (e.g., forensic support, vocational support) to participants.
- Build and maintain relationships with landlords and housing stakeholders to create a network of housing resources for ACT participants.
- Assist participants with accessing and maintaining safe, affordable housing, including applications for and maintenance of housing‑related funding such as HUD subsidies and Section
8. - Facilitate educational and support groups focused on housing‑related skills and needs.
- Provide primary support to ACT participants while cross‑training other team members on housing resources, aiming to promote mental health stability and reduce forensic and hospital recidivism.
- Perform any other duties as assigned.
- Heavy exertional activity: occasionally lift/carry up to 50 lbs.; sit, stand, and walk at least 6 hours in an 8‑hour shift.
- Non‑exertional activity: frequent stooping, crouching, handling, and occasional fine manipulation.
- Functional capacity for heavy, medium, light, and sedentary work.
- Good visual and auditory acuity to provide comprehensive care.
Bachelor's degree with major coursework in criminal justice, sociology, social work, psychology, counseling, or a related field.
Training and Experience- Minimum of two years of direct‑service experience with individuals having mental health concerns.
- Preferred experience working with individuals with forensic involvement and/or co‑occurring substance use disorders.
- Valid driver's license and a driving record in compliance with St. Luke's University Health Network policy.
