Overview
The Multi‑Site Program Specialist is responsible for providing person‑centered case management, program oversight, and regulatory compliance for individuals receiving services across the agency's Day Program. This position manages an assigned caseload across multiple program locations, develops and monitors Individual Support Plans (ISPs), coordinates interdisciplinary services, ensures compliance with Office of Developmental Programs (ODP) regulations, and collaborates with Site Directors to promote quality services, community integration, and positive outcomes.
Regular travel between assigned Day Program locations is required.
Case Management
- Maintain an assigned caseload of individuals receiving services across multiple Day Program locations.
- Conduct comprehensive assessments and develop, implement, monitor, and revise Individual Support Plans (ISPs) in collaboration with interdisciplinary teams.
- Monitor progress toward individual goals and recommend revisions based on changing needs, preferences, and outcomes.
- Ensure services are delivered in accordance with person‑centered principles, individual preferences, strengths, and assessed needs.
- Maintain accurate, timely, and confidential documentation in accordance with agency policies, ODP regulations, and all applicable state and federal requirements.
- Monitor documentation to ensure required assessments, reviews, and program updates are completed within established time frames.
- Coordinate and facilitate Person‑Centered Planning meetings.
- Collaborate with individuals served, families, guardians, Supports Coordinators, medical professionals, behavioral specialists, and other members of the interdisciplinary team.
- Advocate for individual choice, self‑determination, community inclusion, independence, and meaningful outcomes.
- Serve as the primary case management resource for individuals assigned to the caseload.
- Monitor program documentation for quality, accuracy, and regulatory compliance.
- Ensure compliance with agency policies, licensing standards, and applicable Office of Developmental Programs (ODP) regulations.
- Participate in quality assurance reviews, internal audits, licensing inspections, and corrective action initiatives.
- Identify opportunities for program improvement and recommend strategies that enhance service quality and person‑centered outcomes.
- Assist Site Directors in maintaining compliance with documentation, program implementation, person‑centered practices, and applicable regulatory requirements.
- Provide guidance and consultation to Site Directors and Direct Support Professionals regarding implementation of Individual Support Plans.
- Coach staff on documentation standards, person‑centered practices, and regulatory requirements.
- Assist with implementation of behavioral support strategies and individualized programming.
- Serve as a resource to Site Directors and Direct Support Professionals regarding best practices in service delivery.
- Provide direct support to individuals receiving services, including assisting with Direct Support Professional (DSP) coverage to maintain required staffing ratios during staffing shortages or operational needs, as assigned.
- Promote meaningful community participation and inclusion for individuals receiving services.
- Assist individuals in accessing community resources, volunteer opportunities, recreational activities, and integrated community experiences.
- Provide transportation as needed to support community participation and program activities.
- Serve as a liaison between Site Directors, Direct Support Professionals, Supports Coordinators, families, guardians, medical providers, behavioral specialists, and community partners.
- Participate in interdisciplinary team meetings to coordinate services and support positive outcomes.
- Partner with Site Directors to address program needs and ensure continuity of care across multiple Day Program locations.
- Maintain assigned caseload records and required documentation.
- Complete reports, assessments, and documentation within established time frames.
- Participate in agency committees, staff meetings, trainings, and professional development opportunities.
- Assist with special projects and initiatives supporting the Day Program.
This position does not have direct supervisory responsibility.
Education and Experience Required- Bachelor's degree in Human Services, Social Work, Psychology, Rehabilitation, Special Education, or a related field.
- Minimum of two (2) years of experience in human services, case management, or supporting individuals with intellectual and developmental disabilities.
